Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will possess a strong understanding of human physiology and anatomy, with a focus on providing exceptional patient care across various settings in a home health environment. This role is essential in helping patients recover from injuries, manage pain, and improve their overall physical function.

Full time Direct Hire opportunity. 8am to 5pm - Monday to Friday. Flexible schedule.

One Saturday a month required.

Key Responsibilities:

Assessment & Evaluation:

Conduct thorough physical therapy assessments to evaluate patients’ medical, psychosocial, and environmental needs.

Perform initial client assessments, ensuring all necessary authorizations, consents, and intake data are obtained.

Plan of Care Development:

Collaborate with physicians, patients, and the Clinical Manager to develop and implement an individualized Plan of Care based on comprehensive evaluations.

Set realistic and measurable goals in partnership with patients and their caregivers, monitoring and documenting progress.

Qualifications:

Graduate from an accredited physical therapy program recognized by the American Physical Therapy Association or relevant accrediting bodies.

Current Florida physical therapist license and compliance with CEU and HIV licensure requirements.

Previous experience in geriatric care and home health is preferred.

Valid Florida driver’s license, clean driving record, and proof of insurance are required, as travel to client homes is necessary.
