Community Health Educator FULL-TIME | NON-EXEMPT Posted August 29, 2024. JOB SUMMARY The Community Health Educator is responsible for teaching people about behaviors that promote health and wellness in our community. The Community Health Educator will use appropriate strategies, and methods to facilitate health promotion and disease prevention of sexual health. They must understand the diverse populations served by IHB. As well as, a passion for achieving health equity for the people we serve in this community.
ESSENTIAL FUNCTIONS - Plan, organize and deliver the educational components of the Sexual and Reproductive Health Services (SRHS) grant and local intervention activities within the community.
- Educate on family planning, STI screening and prevention through tabling at community events and pow-wows
- Collaborate with community partners to offer education sessions on family planning and STI screening/prevention.
- Collaborate with other IHB staff to offer education sessions related to SRHS at internal events.
- Collaborate with SRHS team to provide education/training for IHB medical staff on gender identity basics.
- Collaborate with community partners to host focus groups related to the grant guidelines.
- Develop EHR to include gender assessment to ensure appropriate cancer screening services, mammograms, and pap smears.
- Track attendance, encounters, participation, and rates of completion of participants.
- Conduct surveys and assessments of programs and events.
- Maintains patient confidentiality and documents (office visits and community) patient education specific to SRHS in the electronic health record.
- Provides excellent customer service to patients, peers, leadership, and community partners.
- Position will work with the Health Programs Manager to collect and compile data for grant reporting.
- Participate in Performance Improvement activities as needed.
- Perform other duties as assigned.
- Position requires occasional evening and weekend work hours to attend local community events and powwows.
KNOWLEDGE, SKILLS, AND ABILITIES - Actively participates and works positively, flexibly, and cooperatively in a team effort to accomplish the goals of the Indian Health Board of Minneapolis.
- Manages time effectively and prioritizes task completion to meet deadlines.
- Knowledge of American Indian cultures and other cultures served by IHB.
- Proficiency with Microsoft Office applications.
- Comfortable speaking in groups or one-to-one sessions.
- Detail-orientated and self-motivated.
- Organizational and clerical skills, including use of PC, Excel, Word and EMR (will train on EMR)
- Ability to show initiative and solve problems, manage projects, be accurate and attentive to deadlines while working independently.
EDUCATION AND EXPERIENCE - Associate's degree in a health-related field or equivalent experience in lieu of degree.
- Valid driver's license with good driving record.
- Experience working with the Minneapolis American Indian community, a plus.
HIPAA IHB makes reasonable efforts to limit access to and use of protected health information (PHI) by employees to the minimum necessary performance of assigned duties as outlined in job descriptions. This position is access restricted to PHI needed to provide health education and carry out health care operations in assigned clinical area.
PHYSICAL DEMANDS The work requires some physical exertion such as long periods of standing, walking, and recurring bending, crouching, stooping, stretching, reaching, or similar activities. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood borne pathogens, physical and chemical hazards.
